Position Title: Direct Support Professional (DSP) – 245D Waivered Services

Company: Hennepin Care LLC

Location: Twin Cities, Minnesota

Reports To: Program Supervisor

Position Summary

The Direct Support Professional (DSP) provides individualized support to people receiving 245D

Waivered Services. Support is delivered based on each person's Coordinated Service and

Support Plan (CSSP) and CSSP Addendum, with a focus on promoting personal choice,

independence, inclusion, and safety.

Essential Duties and Responsibilities

● Provide direct support in accordance with each person's CSSP and CSSP Addendum

● Assist individuals with daily routines, independent living skills, and personalized goals

● Coordinate access to appointments, community outings, and other meaningful activities

● Support participation in social, recreational, or volunteer opportunities

● Promote informed choice and person-centered decision-making

● Document services provided and progress toward identified outcomes

● Maintain a respectful, clean, and safe environment

● Protect individual rights, including privacy, dignity, and autonomy

● Respond to incidents and follow emergency procedures appropriately

● Communicate effectively with team members, supervisors, and other support

professionals

Requirements and Qualifications

● Must be at least 18 years old

● High school diploma or GED

● Ability to pass a Minnesota Department of Human Services (DHS) background study

● Strong verbal and written communication skills

● Ability to maintain confidentiality and follow data privacy laws (HIPAA)

● Reliable and dependable with the ability to work independently and as part of a team

● Must be able to complete documentation accurately and on time

● Valid driver's license and reliable transportation (if required for the role)

● Physical ability to perform job duties, including light lifting, walking, or standing for

extended periods

Job Types: Full-time, Part-time

Pay: $ $18.00 per hour
