WHO YOU CAN BECOME: The Community-Based Service Provider will be working with youth and families during and after placement to help reduce the length of stay in a residential facility now and to help prevent future out of home placement. Some of the services provided by this position could include, but are not limited to:
- After Care Services
- Community-Based Services through Community Psychiatric Supports and Treatment (CPST) and Psychosocial Rehabilitation (PSR) Services
- Service array includes Intensive Interventions, Crisis Avoidance, Intermediate Term Crisis Management, Rehabilitative Psychoeducation, Strengths-Based Service Planning, Rehabilitative supports, Personal and Community Competence, Social and Interpersonal Skills, Daily Living Skills, and Community Integration activities.
LaSalle School is seeking part-time and per diem providers. The part-time position requires a minimum of 10 hours per week of which 8 hours must be with clients and the per diem position requires a minimum of 5 hours per week of which 4 hours must be with clients. Hours worked above the minimum must include at least 80% of the hours with clients. The Community-Based Service Provider provides services to multiple counties in NYS and will require driving your personal vehicle of up to an hour to meet with youth and families. Personal vehicle usage is reimbursed at the IRS mileage reimbursement rate. Must be available to travel to the following areas: Ulster, Dutchess, Sullivan, Greene and Columbia counties.
WHAT YOU'LL DO:
- Collaborate with family and service providers to identify goals and objectives, and establish and implement the Treatment Plan, based on the child and caregiver's needs and preferences.
- Coordinate with the child and the caregiver on scheduling and provision of services.
- Complete all required documentation within required time frames.
- Facilitate service provision and attend relevant collaborative meetings as required.
- Share information related to delivery of services with stakeholders as required.
- Provide comments and information to collateral contacts for the purpose of Treatment planning, assist in tracking progress.
- Attend all required training.
- Meet regularly for clinical supervision.
WHAT YOU NEED:
- Bachelor's degree in Human Services, behavioral health fields, or another related field.
- Must have 2-3 years of experience post-bachelor's degree in a behavioral health and/or human services field.
- Must have a valid and clean NYS driver's license as determined by the agency.
- Flexibility of work hours (evening and weekends hours).

- Possess excellent communication with strong interpersonal skills, computer skills, and clinical skills (including assessment, engagement, and therapy).
- Able to manage multiple tasks, set priorities, and manage deadlines.
- Knowledge of community-based services.
- Able to work as a team member and effectively use crisis management skills.
WHAT WE OFFER:
- Part-time employees start at 5 days off for vacation, and 13 paid holidays a year. Both part-time and Per Diem employees earn 12 sick days per year and other leave time available dependent on the circumstances.
