Home Health agency is inviting you to join our dedicated healthcare team as an LVN (Licensed Vocational Nurse), RN (Registered Nurse), or CHHA (Certified Home Health Aide) in and . • Deliver comprehensive patient care by performing physical examinations, vital sign assessments, and health evaluations using advanced medical terminology and physiology knowledge.
• Administer medications and injections accurately following HIPAA regulations and medical documentation standards.
• Assist with complex procedures such as IV infusions, catheterizations, wound suturing, and sterile processing techniques.
• Manage airway support including ventilator management and airway maintenance for patients with acute or chronic respiratory needs.
• Conduct diagnostic evaluations utilizing EMR (Electronic Medical Records) systems like Epic, Cerner, or Athenahealth to document patient information precisely.
• Provide specialized care in areas such as dementia care, Alzheimer's care, memory care, hospice support, and behavioral health interventions.
• Perform patient assessments including intake procedures, specimen collection/processing, and physical examinations to ensure accurate diagnosis and treatment planning.
• Support discharge planning by coordinating post-acute care services and educating patients on medication administration, nutrition, and self-care strategies.
• Collaborate with multidisciplinary teams in emergency medicine, ICU (Intensive Care Unit), Level I Trauma Centers, Level II Trauma Centers, neonatal units with ventilator support, PICU (Pediatric Intensive Care Unit), and surgical settings like the OR (Operating Room).
• Engage in triage activities for urgent care or outpatient visits to prioritize patient needs effectively.
• Maintain strict infection control protocols including aseptic techniques during procedures like spinal taps or gastric lavage.
• Document all patient interactions thoroughly in medical records ensuring compliance with HIPAA standards and ICD coding requirements such as ICD-9 or ICD-10 classifications.
• Educate patients and families on health coaching topics including nutrition, family planning, pain management, and working with people with disabilities or developmental challenges.

Experience

• Proven experience in diverse clinical environments, preferably home health agencies .
• Experience administering injections (including IV insertion), phlebotomy/venipuncture skills required for accurate specimen collection.
• Knowledge of medical imaging procedures such as sonography or radiology is a plus.
• understanding of anatomy and physiology principles to conduct thorough patient assessments and diagnostic evaluations effectively.
• Demonstrated ability to work independently while collaborating seamlessly within multidisciplinary teams across inpatient and outpatient settings.
