Responsibilities:
Key Responsibilities: Conduct comprehensive nutritional assessments for patients with ALS or Diabetes and other chronic diseases; Develop and implement personalized nutrition care plans based on clinical status, lab values, and patient preferences; Monitor and adjust nutrition interventions to support disease progression, metabolic control, and quality of life; Collaborate with physicians, nurses, speech-language pathologists, and other allied health professionals; Educate patients and caregivers on dietary modifications, feeding techniques, and use of nutritional supplements or enteral feeding; Document all assessments, interventions, and outcomes in the electronic medical record (EMR) system; Stay current with evidence-based practices and emerging research in neuromuscular and metabolic nutrition; Proficiency in managing enteral nutrition and dysphagia diets; Excellent communication and counseling skills. Perform professional consultation and diabetes educational services for USA Health; assist in development and implementation of educational programs, including policies and procedures for staff, patients, and families/caregivers that meet American Diabetes Association (ADA) Recognized Program standards of care and health education guidelines, in collaboration with Endocrinologist, Providers, program staff, and other healthcare professionals; provide outpatient education and motivational health coaching to type I, type 2, and gestational diabetes patients individually and in group sessions as needed or as directed by Provider on topics including but not limited to: diabetes and disease process, medication/insulin and diabetes injectable administration, nutrition therapy/weight control, glucose monitoring and continuous glucose monitoring technology, physical activity, problem solving treatment and prevention of hyperglycemia/hypoglycemia/DKA/HHNK/sick days, prevention and care of complications, foot care, coping with diabetes, and behavioral goal setting; document assessments, interventions, and plans with approved forms and policies; provide and maintain ongoing patient assessment and follow-up for patients/families which may include: utilizing continuous glucose monitoring reports when available, monitoring/evaluating short and long-term patient responses to therapeutic interventions in collaboration with Providers, making recommendations to Provider, and initiating adjustments in the plan of care as required; enter outcome data for all patients seen individually and on follow-up in ADA Chronicle database; assist in evaluating effectiveness of services through continuous quality improvement measures; assist in organizing and facilitating annual Diabetes Advisory Committee meeting; assist in conducting ADA program annual review; train and educate nursing staff in diabetes and clinical procedures; coordinates in-service and continuing education, and other related diabetes training; develop collaborative relations with other departments, services, and community health care agencies to facilitate and support quality diabetes care; ensure that Provider referral orders are documented and charge patient insurance according to Physician Group policies; maintain professional growth and development through seminars, workshops, and professional affiliations to keep abreast of latest trends; professional knowledge and skill as a diabetes and health educator; In-depth knowledge and understanding of the nature of diabetes and related issues and patient care concerns; ability to investigate and analyze information and draw conclusions; ability to design, develop, deliver, and evaluate educational programs and materials; ability to interact with patient care professionals as part of an interdisciplinary team; ability to provide technical advice, guidance, and support to professional staff; strong interpersonal and communication skills and the ability to work effectively with a wide range of constituencies In a diverse community; regular and prompt attendance; ability to work schedule as defined and overtime as required; related duties as required.
Qualifications:
Bachelor's degree in nursing from an accredited institution as approved and accepted by the University of South Alabama , three years of professional nursing experience, and licensure with the state of Alabama as a registered nurse; OR a bachelor's degree with major coursework in human nutrition, food and nutrition, dietetics, or food systems management from an accredited institution as approved and accepted by the University of South Alabama, three years of professional dietitian experience, registry with the Commission on Dietetic Registration and licensure with the state of Alabama as a dietitian. Certified Diabetes Educator is preferred. MSN Highly preferred. Experience in diabetes care and chronic disease program management is preferred.
