About the Role

The Enrollment Specialist drives member growth in CareCollab's Health Home and Social Care Network (SCN) programs through proactive, community-based outreach. You'll connect with eligible individuals, explain program benefits, verify eligibility, and guide them through enrollment — both in the field and by phone.

This is a high-energy, people-facing role perfect for someone who thrives on outreach, relationship-building, and meaningful impact. You'll use your communication and persuasion skills to help individuals access critical care and support.

What You'll Do

  • Conduct high-volume outreach (calls, visits, and community events) to identify, engage, and enroll eligible members.
  • Present program benefits clearly and persuasively to diverse communities.
  • Verify eligibility, obtain signed consents, and complete documentation in JotForm, and Foothold Care Management (FCM) EMR.
  • Collaborate with Intake and Enrollment teams to meet and exceed enrollment goals.
  • Participate in community engagement events to promote visibility and referrals.

What You Bring

  • 1+ year of experience in outreach, sales, enrollment, or community engagement
  • Strong communication and public engagement skills — comfortable pitching to strangers
  • Resourceful, results-driven, and resilient; thrives in fast-paced outreach environments
  • Excellent organization, follow-up, and time management
  • Tech-savvy with EMRs, CRMs, and digital outreach tools
  • Bilingual (Spanish strongly preferred); familiarity with NYC communities a plus
