Resident Care Coordinator

We're searching for a compassionate and detail-oriented Resident Care Coordinator (RCC) to join our leadership team and help us deliver exceptional care to the wonderful people who call Birch Creek home.
As our Resident Care Coordinator, you'll be at the heart of our care team-overseeing the daily delivery of personal care services, supporting and training our caregivers, and ensuring that every resident receives thoughtful, individualized support. You'll work closely with our nurse and administrator to keep operations running smoothly, making sure that resident needs are met and that our community stays in full compliance with state regulations.
A typical day in this role involves coaching and mentoring CNAs, coordinating resident assessments and service plans, scheduling care staff, and maintaining open, supportive communication with residents and their families. You'll play a vital role in hiring and onboarding new team members, monitoring quality of care, assisting with medication program audits, and ensuring that our team consistently provides care that meets or exceeds expectations. You'll also respond to resident needs, assist with activities of daily living when necessary, and serve as a reassuring, trusted point of contact for families and staff alike.
We're looking for someone who is not only clinically skilled but also kind, organized, and motivated to lead with heart. The ideal candidate will have at least two years of experience working with seniors in assisted living or healthcare settings, a current CNA license, and certifications in CPR and First Aid. A passion for mentoring others, strong communication skills, and a genuine love for seniors are essential.
