Job Summary

As a Wound Care RN in the Med/Surg setting, you'll be the go-to pro for all things wound, ostomy, and continence care. You'll assess, plan, and implement care strategies that promote healing and improve patient outcomes. This role blends hands-on clinical expertise with education, collaboration, and a dash of creative problem-solving.

Roles & Responsibilities

• Conduct focused health assessments related to wound, ostomy, and continence care.
• Develop and implement individualized plans of care that promote healing and comfort.
• Assess and document patient responses to wound care treatments and adjust plans accordingly.
• Work collaboratively with the healthcare team to ensure continuity and quality of care.
• Educate patients, families, and staff on wound prevention, ostomy management, and best care practices.
• Provide staff training and participate in ongoing education programs.
• Evaluate the quality of care delivered by clinical staff and contribute to continuous improvement initiatives.
• Support unit and hospital-wide quality improvement projects and nursing initiatives.
• Maintain strict confidentiality and data security in compliance with patient rights policies.
• Stay up to date with wound care best practices, products, and technologies.

Qualifications

• Minimum 2 years of Med/Surg experience.
• Strong Wound and Colostomy experience required.
• Excellent clinical judgment, documentation, and communication skills.
• Ability to work independently and as part of a team.

Education

• Associate's Degree (ASN or ADN) from an accredited Nursing Program required.
• Bachelor's Degree (BSN) preferred.

Required Certifications & Licensure

• Active New York State RN License.
• AHA BLS certification.
• Primary Source Verification from the NYS Department of Education (NYSED).
• One of the following wound certifications:
• Certified Wound Ostomy Continence Nurse (CWOCN) – WOCNCB
• Certified Wound Ostomy Nurse (CWON) – WOCNCB
• Certified Wound Care Nurse (CWCN) – WOCNCB
• Certified Wound Specialist (CWS) – ABWM
• Wound Care Certified (WCC) – NAWCO

Knowledge and Skills

• In-depth understanding of wound, ostomy, and continence management.
• Familiarity with evidence-based wound care protocols and products.
• Strong organizational, assessment, and teaching skills.
• Compassionate communication and cultural sensitivity.
• Ability to think critically and act decisively under pressure.
