We are seeking a dedicated, client-focused Life Skills Trainer (LST) to join our team on a PRN basis with consistent weekly part-time hours.

This role is perfect for someone who thrives on making a difference, enjoys working independently, and values strong professional relationships. You'll work directly with individuals in their homes and communities, helping them develop practical life skills that lead to lasting independence.

What You'll Do

As a Life Skills Trainer, you will:

  • Provide one-on-one training and coaching to clients transitioning to independent living.
  • Teach and support a wide range of essential life skills, including:

Self-care and hygiene

Communication and interpersonal skills

Budgeting and money management

Community mobility and transportation skills

Time management, scheduling, and organization

Goal setting and decision-making

Home and household maintenance

Emergency preparedness

Advocacy and self-advocacy skills

  • Accurately document session details and client progress in compliance with Medicaid Waiver Program requirements.
  • Collaborate with clients and team members to ensure the highest level of client-centered care.

About the Program

Our Life Skills Training (LST) program serves Colorado's Medicaid population, supporting individuals as they transition from institutional settings to home and community-based living. The program's goal is to enhance physical, emotional, and financial independence, empowering clients to thrive within their communities.

  • Holds a bachelor's degree in human services, social work, psychology, or a related field (required).
  • Experienced in working with individuals with disabilities or those transitioning to independent living (preferred).
  • Able to successfully pass a background check and drug test.

Physical Demands

This position requires the ability to:

  • Sit, stand, walk, and occasionally lift/push/pull up to 25 lbs.
  • Communicate clearly and effectively with clients and team members.
