General Functions:

  • Investigates and analyzes potential and actual professional liability and general liability exposures in

the Health System; evaluates the extent and elements of exposure and recommends appropriate actions for risk mitigation.

  • Investigates evaluates and documents pre-litigation occurrences and claims. Recommends resolution, and completes negotiation of resolution within authority granted by Director or Risk Management. Completes notifications and financial recommendations needed for compliance with tracking and insurer requirements, including insurance carrier notifications, reserve recommendations, and any federal, state, or entity reporting requirements (including SMDA, DataBank, etc.) as necessary.
  • Initiates Root Cause Analysis (RCA) Process within 72 hours of a Serious Safety Events/Sentinel Events (SSE/SE) requiring an RCA.
  • Submits RCA for review to Regional Director Risk Management within 14 days of the event.
  • Completes all paperwork and presents to Home Office or other regulatory body within 30 days of the event.
  • Assists the Leadership in ensuring RCA action plans are implemented and complied with.
  • Maintains an up to date list of all RCA's and Intensive Analysis's ( IA's) in process along with date completed and actions taken.
  • Maintains MIDAS coding to include RCA, IA, and Common Cause Analysis (CCA), to include the appropriate severity level, Generally Accepted Performance(GAP's,) and Inappropriate Act classifications.
  • Maintains MIDAS coding to include Complaint or Grievance as appropriate along with required letter follow up.
  • Reviews, codes and investigates occurrence reports; recommends corrective actions based on individual reports or trends. Provides quality and peer review referrals as appropriate.
  • Provides inservice education on risk management and patient safety topics, both standard basic education and complex focused topics as needed and/or requested.
  • Provides on-call assistance for emergent/urgent risk management issues to all BHS facilities as needed. On-call duties are assigned on a rotation basis when possible.
  • Develops and implements Proactive Patient Safety Tools
  • Reviews data and develops preventative measures or actions to address patient safety trends.
  • Facilitates or participates in Proactive Risk Assessments ( PRA's) formerly called Failure Modes Effects Analyses (FMEA's) to look for proactive improvements
  • Utilizes resources efficiently and effectively
  • Maintains safe environment
  • Participates in Performance Improvement Activities.

This position may qualify for a sign-on bonus.

POSITION SUMMARY:

The Patient Safety Officer will have primary oversight of the facility-wide patient safety program. This leadership role will direct others within the facility towards process improvements to support the reduction of medical/health care errors and other factors that contribute to unintended adverse patient outcomes. This practitioner provides leadership for safety assessments, coordinates the activities of the patient safety function or committee, educates other practitioners on the system-based cause for medical errors, consults with management and staff, and communicates literature-based ideas regarding effective patient safety strategies to others within the organization. Responds to complaints and grievances as required by regulatory bodies. Acts as a Patient Safety Liaison and functions as a patient safety expert and assists leaders and staff in proactive patient safety activities. Provides basic and complex preventive risk management assessment of processes, procedures, and programs, including in-service education, consultation, liaison activities, and on-call emergency assistance to providers. Completes established competencies for the position within designated introductory period. Other related duties as assigned.

MINIMUM EDUCATION: Bachelor's Degree from an accredited college in healthcare based discipline.

PREFERRED EDUCATION: Master's Degree or J.D. from an accredited college or university in a healthcare related field. Completion of patient safety curriculum through ASHRM, NPSF, AHA, IHI, or another academic provider.

MINIMUM EXPERIENCE: 5 years in direct patient care and 3 years in quality/risk/patient safety.

PREFERRED EXPERIENCE:

REQUIRED CERTIFICATIONS/LICENSURE: CPHRM or CPPS Certification or eligible to take the exam within two years of employment.

PREFERRED CERTIFICATIONS/LICENSURE: Associate in Risk Management or Certified Professional in Healthcare Risk Management desirable. License in a clinical discipline.
