Elevate Therapy Consulting Group is seeking a highly motivated and experienced Licensed Practical Nurse (LPN) or Registered Nurse (RN) to join our team as a Full-Time MDS Education and Training Specialist.

Role Overview:

In this pivotal position, you will provide expert training, oversight, and support to healthcare facilities to ensure optimal performance in MDS completion, PDPM compliance, and comprehensive care planning. The ideal candidate will utilize their clinical knowledge and educational skills to enhance documentation accuracy, improve resident outcomes, and ensure adherence to regulatory standards.

Key Responsibilities:

  • Deliver PDPM, MDS, and care plan training to new MDS Coordinators.
  • Oversee facility PDPM and triple-check meetings.
  • Track and monitor each facility's effective use of tools that enhance MDS coding accuracy and reimbursement processes.
  • Conduct educational sessions for MDS and interdisciplinary team (IDT) members, including frontline staff, on MDS, PDPM, Quality Measures, and other CMS-related regulations.
  • Monitor and analyze QM and QRP performance on a monthly and quarterly basis, providing data analysis and follow-up training as needed.
  • Review facility documentation to analyze clinical areas for improvement and establish training programs to address key issues.
  • Assist in monitoring MDS calendars to ensure timely completion of all MDS assessments, including PPS Medicare, quarterly, annual, and significant change assessments.
  • Facilitate interdisciplinary participation in completing the Minimum Data Set (MDS) for each new admission, ensuring compliance with regulatory time frames. Provide education to ensure thorough and complete documentation as required by federal, state, and medical standards.
  • Oversee the completeness and accuracy of documentation related to MDS and care planning per federal and state standards.
  • Review resident care plans for comprehensiveness and individualization, offering follow-up education and guidance as needed.
  • Maintain confidentiality of all resident care information, including protected health information.
  • Assist with ICD-10 coding for residents and provide staff education on coding as necessary.

Requirements:

  • Experience with RAI, MDS, care planning, and PDPM, as well as familiarity with Nebraska Medicaid CMI.
  • Preferred experience with iQIES, quality measures, and the Quality Reporting Program.
  • Licensed as an LPN or RN.
