ART Patient Care Coordinator
Job Summary
- The ART Patient Care Coordinator navigates individuals through HIV and related health and psychosocial needs; arranges appointments for individuals with HIV; and provides patient education.
General Responsibilities
- Navigate patients through their ART care plan in accordance with the ART navigation procedure document, including educating patients on ART point of care and expectations of HIV treatment visits, HIV basics, HIV transmission and U=U, HIV disclosure laws, and adherence to HIV medical care, etc. .
- Document progress notes and referred/rendered services in the patient's chart in a timely manner.
- Refer patients to Insurance or Financial Aid for assistance when needed, and ensure patients are aware of a wide range of resources that can address needs outside the scope of MCPC.
- Participate in weekly ART team meetings and bi-weekly provider meetings to assess and address ongoing needs of patients.
- Assist Communications and Prescription Management teams in ensuring regular contact is established with patients and monitored at regular intervals through protocols and processes.
- Evaluate Ryan White eligibility and refer to ART Care Manager or outside agencies for enrollment assistance.
- Oversee transportation of patients to appointments and ensure access to key services as needed.
- Engage in continuing education about HIV and key population issues to render services with cultural humility, and instruct ART team on best practices.
- Accurately report hours worked in ADP, keep supervisor informed of any deviations from normal work schedule, and take lunch breaks in accordance with state laws and Diversity Health policies.
- Completes special projects and other duties as assigned by supervisors.
- Accept and respond to ART-related calls that come in through RingCentral during work hours.
- Monitor widgets within the appropriate departments in Bwell, and add and remove explanations and statuses accurately and timely in Bwell.
Job Qualifications
- Ability to work effectively with medical providers, staff, volunteers, patients, the public and external agencies
- Skills in gathering and interpreting data in a medical office setting
- Ability to take initiative and to exercise independent judgment
- Working knowledge of all navigation departments and tools necessary to perform the navigation roles.
- Decision-making and problem-solving expertise
- Ability to maintain appropriate professional boundaries with patients
- Proficient in the use of spreadsheets, databases, data-entry, and general office processing software, such as Microsoft Office, Google, etc..
