Role Description:
We are seeking a dedicated and skilled
Physical Therapist
to join our Interdisciplinary Team (IDT) in providing high-quality restorative and rehabilitative care to elderly and frail participants. This full-time role offers the opportunity to make a meaningful impact through personalized care—without the pressures of high-volume therapy environments.
This position emphasizes long-term participant relationships, collaboration with a supportive clinical team, and a balanced, sustainable work environment.
Key Responsibilities:
- Conduct comprehensive physical therapy assessments to identify participant needs, preferences, and goals
- Participate in IDT meetings to develop individualized and integrated care plans
- Deliver and document therapy interventions, including active/passive exercise, gait training, ADL support, and therapeutic modalities (massage, heat, water, sound, light, electricity)
- Provide supervision, training, and guidance to Physical Therapy Assistants (PTAs)
- Educate participants, caregivers, and staff on therapy techniques and proper equipment use
- Support ongoing quality improvement and contribute to a high standard of clinical excellence
Qualifications:
- Bachelor's degree in Physical Therapy
- Valid California Physical Therapy License (or eligibility to obtain one)
Preferred:
- Master's or Doctoral degree in Physical Therapy
- Minimum 2 years of clinical experience, ideally with elderly or medically complex populations
- Experience in outpatient, acute care, inpatient, or skilled nursing settings
- Current CPR certification
