Role Description:

We are seeking a dedicated and skilled
Physical Therapist
to join our Interdisciplinary Team (IDT) in providing high-quality restorative and rehabilitative care to elderly and frail participants. This full-time role offers the opportunity to make a meaningful impact through personalized care—without the pressures of high-volume therapy environments.

This position emphasizes long-term participant relationships, collaboration with a supportive clinical team, and a balanced, sustainable work environment.

Key Responsibilities:

  • Conduct comprehensive physical therapy assessments to identify participant needs, preferences, and goals
  • Participate in IDT meetings to develop individualized and integrated care plans
  • Deliver and document therapy interventions, including active/passive exercise, gait training, ADL support, and therapeutic modalities (massage, heat, water, sound, light, electricity)
  • Provide supervision, training, and guidance to Physical Therapy Assistants (PTAs)
  • Educate participants, caregivers, and staff on therapy techniques and proper equipment use
  • Support ongoing quality improvement and contribute to a high standard of clinical excellence

Qualifications:

  • Bachelor's degree in Physical Therapy
  • Valid California Physical Therapy License (or eligibility to obtain one)

Preferred:

  • Master's or Doctoral degree in Physical Therapy
  • Minimum 2 years of clinical experience, ideally with elderly or medically complex populations
  • Experience in outpatient, acute care, inpatient, or skilled nursing settings
  • Current CPR certification
