Summary: The Registered Nurse (RN) will provide specialized medical training to staff members at Your Choice Home Care (YCHC). This role ensures that all YCHC employees are equipped with the necessary skills and knowledge required to provide high-quality care to individuals with developmental disabilities and medical concerns. RN will conduct trainings that pertain to specialty needs such as but not limited to: Diet, Specialized treatment or medication (Epi Pen, Diastat, etc.), Glucose Monitoring, Blood pressure monitoring.

RN services provided will include, as required, in-home visits with individuals served, guardians, and/or YCHC staff.

Responsibilities of Service Provider:

· Manage YCHC employee training and compliance for individuals with medical needs including identification, design, and implementation of specialized medical training for YCHC staff.

· Review of Medical Examination Reports and completion of required documentation including Addressing Enhanced Needs (AEN) forms.

· Collaborate with YCHC leadership to ensure that training programs are aligned with organizational goals and healthcare standards.

· Work collaboratively with YCHC team members on new admission intakes as recommended by the CEO or Designee with receipt of required health care information for prospective candidates.

· Receive and review Physician Visit Records from and provide clinical feedback and recommendations.

· Follow up as necessary to assure prompt, quality, and safe care.

· Collaboration and cooperate with DDD/DHS for any investigations.

Requirements of Service Provider:

· Bachelors of Science in Nursing or equivalent and active nursing licensure in the State of NJ

· Certification as a CPR and First Aid Trainer through the American Heart Association, Red Cross, or a similar organization.

· Knowledge of healthcare compliance regulations, including HIPAA, Medicaid, and DDD standards.

· Successful completion of initial, and ongoing, Background and Exclusion Checks, fingerprinting, drug testing, and other pre-employment screening, as applicable to comply with NJ DDD Supports Program or Community Care Program Policy & Procedures Manuals.

· Current drivers license and clean driving record.

Job Types: Part-time, Travel nursing, Per diem

Pay: $85.00 per hour

Expected hours: No less than 10 per week
