Intake Clinical Review

We're looking for that extra motivated LVN to help the Intake Team.

Responsibilities:

  • Clinically review all referrals to re-assured they eligible for home health care.

-Work closely with the intake department and referral team.

- Collaborate with healthcare professionals to identify areas for improvement in patient care and outcomes

- Develop and implement performance improvement initiatives and strategies

- Analyze data and metrics to evaluate the effectiveness of improvement efforts

- Provide education and training to staff on performance improvement processes and best practices

- Conduct audits and assessments to ensure compliance with regulatory standards and guidelines

- Facilitate interdisciplinary team meetings to discuss performance improvement goals and progress

- Participate in quality improvement committees and initiatives

- Stay up-to-date with industry trends and advancements in performance improvement methodologies

Requirements:

- Strong knowledge of medical terminology, case management, and medical management principles

- Familiarity with Medicare regulations and guidelines

- Understanding of anatomy and physiology

- Knowledge of HIPAA regulations and patient privacy practices

- Experience working as a nurse or in a healthcare setting preferred

- Proficiency in using electronic health record systems, such as Epic

- Excellent communication and interpersonal skills

- Ability to work independently and collaboratively in a team environment

- Strong analytical and problem-solving abilities

Pay: $55, $60,000.00 per year
