Key Responsibilities

  • Deliver individualized physical therapy services in a 1:1 setting, promoting patient independence and functional progress.
  • Develop and implement customized care plans aligned with each patient's goals and clinical needs.
  • Collaborate with an interdisciplinary healthcare team to ensure coordinated and comprehensive care.
  • Maintain accurate and timely documentation of assessments, treatments, and outcomes.
  • Educate and motivate patients and caregivers to support ongoing recovery and long-term wellness.
  • Participate in professional development and apply innovative, evidence-based practices in clinical care.

Requirements

  • Active Physical Therapy license in the state of practice, or eligibility to obtain one.
  • Degree from an accredited Physical Therapy program.
  • Basic computer proficiency, including experience with electronic health record (EHR) systems.
  • Current CPR certification.

Preferred Qualifications

  • Experience or strong interest in geriatric rehabilitation or home-based care.
  • Excellent communication, interpersonal, and organizational skills.
  • Demonstrated commitment to compassionate, patient-centered care.
