Overview

We are seeking a highly skilled and compassionate Nurse Practitioner to join our healthcare team. The ideal candidate will possess extensive clinical expertise across various specialties, including critical care, emergency medicine, geriatrics, pediatrics, and outpatient care. Experience with EMR/EHR systems is essential. The Nurse Practitioner will play a vital role in providing comprehensive patient assessments, managing acute and chronic conditions, and supporting multidisciplinary teams in delivering exceptional healthcare services. Knowledge of medical documentation, HIPAA compliance, and advanced procedures such as suturing, IV infusion, catheterization, and telehealth is required. This position offers an opportunity to work in diverse settings including hospitals, clinics, assisted living facilities, and home care environments.

Duties

  • Conduct thorough physical examinations and patient assessments across various age groups including pediatrics, geriatrics, neonatal, and adult populations.
  • Manage acute pain and chronic conditions utilizing advanced clinical skills such as suturing, IV insertion, catheter care, and airway management.
  • Perform diagnostic evaluations including laboratory specimen collection/processing, sonography, medical imaging interpretation, and gastroscopy procedures like gastric lavage.
  • Administer injections, dermal fillers, botulinum toxin treatments, vaccines, and other medications following sterile techniques.
  • Develop individualized treatment plans incorporating pharmacological management with CPT coding knowledge and medication administration expertise.
  • Support Home Health care through discharge planning, case management, utilization review, and health coaching aligned with NCQA standards.
  • Collaborate with specialists in areas such as cardiology (cardiac catheterization), ophthalmology (ophthalmology procedures), infectious disease care, hospice & palliative medicine, mental health (behavioral health), and occupational health.
  • Utilize EMR/EHR systems to document patient encounters accurately while ensuring HIPAA compliance.
  • Provide telehealth consultations utilizing telemedicine platforms to expand access to care.
  • Participate in clinical research projects or quality improvement initiatives aimed at enhancing patient outcomes.
  • Educate patients on disease prevention strategies including nutrition counseling and family planning.
  • Engage in post-acute care activities such as discharge planning for patients transitioning from hospital to home or assisted living facilities.

Requirements

  • Valid Nurse Practitioner license with current certification to practice independently in the state of employment.
  • Proven experience in diverse clinical settings such as hospital medicine (Level I & II trauma centers), ICU/PICU/Neonatal ventilator management, emergency room/urgent care environments, nursing homes or assisted living facilities.
  • Strong proficiency with EMR/EHR systems and familiarity with ICD-9/10 coding is preferred.
  • Experience with telehealth platforms for remote patient management is desirable.
  • Knowledge of Workers' Compensation law applicable to occupational health cases is advantageous.
  • Ability to work collaboratively within multidisciplinary teams while maintaining high standards of patient safety and confidentiality under HIPAA regulations.
  • Excellent communication skills with the ability to educate patients across diverse populations, including patients with developmental disabilities or dementia patients requiring memory care support.
