Nana Grants is looking for an exceptional Case Manager (part-time; 20 hours per week) to support student parents in our program from their in-take session through college graduation.

Nana Grants' motto is "Child Care + College = Economic Mobility." We cover 100% of the cost of child care through graduation for low-income student mothers attending an accredited college or university in Georgia. Our mission is to support low-income single mothers pursuing post-secondary education while ensuring their children receive the benefits of quality early childhood education.

The case manager will support student parents from program in-take through college graduation. During their time with Nana Grants, most of our students will experience challenges such as housing insecurity, food insecurity, under-employment or job loss. Nana Grants uses scheduled quarterly and ad hoc case management check-in calls to ensure that students are meeting program requirements and assist them with wrap-around support referrals. Utilizing our risk-based case management tracking system (Salesforce Nonprofit Cloud), the case manager will gather program data to assist with better understanding and serving the needs of our students.

The ideal candidate for this position has at least two years of experience working with socioeconomically at-risk families in metro Atlanta and facilitating referrals to other service providers for coordinated support. Familiarity with state programs such as CAPS, SNAP and Medicaid is preferred. Experience working with at-risk college students is a plus.

Role and Responsibilities

  • Assist with on-boarding of new students by providing case management orientation, introducing yourself and building a foundation for ongoing case management support.
  • Manage a caseload of approximately 50 college students.
  • Conduct quarterly case management calls with current students including detailed note taking and risk tracking in Salesforce, create a service plan for students deemed at risk, and provide wrap-around support referrals.
  • Help students resolve general obstacles that come up throughout the school year by coaching them on self-advocacy and resourcefulness skills, and communicating with school offices to advocate for students.
  • Attend weekly calls with the Executive Director to discuss cases.
  • Respond to all student text/calls/emails within 2 business days.
  • Assist in gathering and reviewing end-of-semester grade transcripts. Confirm that students are meeting program requirements and making adequate progress toward their expected graduation date. Log case notes in Salesforce. Update student records as needed. Report to Executive Director on at-risk cases.
  • Conduct pre-graduation off-boarding calls with graduating students. Prepare students to take over child care tuition payments and coordinate final payment dates with the Executive Director.
  • Maintain program-related data integrity by updating student, provider and payment records as-needed.
  • Assist with program-related data reporting for fundraising and donor reports.
  • Collaborate with the Executive Director to continuously improve operations and program delivery as Nana Grants expands its capacity.

Qualifications include (but are not limited to):

  • Must have a bachelor's degree from an accredited college or university in the area of behavioral or social sciences, social work, or psychology, childhood education and 2 years of paid work experience in the field of social services or human service delivery.
  • Demonstrate organizational, time management, and task prioritization skills
  • Excellent written and verbal communication skills
  • Must have a genuine passion for helping students identify challenges and navigate resources with dignity and respect
  • Experience developing trusting relationships with students, and coaching them toward goals
  • Detail-oriented attention to maintaining accurate and timely case notes
  • Salesforce Nonprofit Cloud experience preferred
  • Have a clear criminal background check
