Mental Health Clinician / Community Outreach Therapist (LCSW, LMHC, LMFT)
Location: New York, NY
Salary: $76,900 per year
Schedule: Monday–Friday, 9 AM–5 PM or 1 PM–9 PM

About the Role
This is an outreach-focused, field-based clinical position serving adults with significant behavioral health needs. You will meet clients where they are: shelters, hospitals, community programs, or public settings to deliver therapeutic support, crisis intervention, and linkage to ongoing services. Your work helps stabilize individuals during vulnerable moments and connects them to long-term treatment, housing, and safety.

Primary Responsibilities
- Provide mental health assessments, brief therapy, crisis stabilization, and clinical support in community and field settings
- Build rapport with clients through ongoing engagement, outreach, and follow-up
- Support transitions from hospitals, shelters, and emergency placements
- Coordinate services with hospitals, housing programs, shelters, and community partners
- Facilitate therapeutic or psychoeducational group sessions
- Complete documentation in the electronic health record (EHR)
- Participate in clinical meetings and contribute to program development and strategy
What You Bring
- Active New York State license: LCSW, LMHC, or LMFT
- Master's degree in Social Work, Mental Health Counseling, Psychology, or a related field
- Experience in community mental health, crisis work, homeless services, or outreach preferred
- Understanding of NYC behavioral health and housing systems
- Strong communication and documentation skills
- Bilingual clinicians highly encouraged to apply
