Job Title: Certified Occupational Therapy Assistant (COTA) – Per Diem

Agency: Provide Home Health Inc.

Location: Los Angeles, CA and surrounding service areas

Compensation: $65 per completed visit

Reports To: Supervising Occupational Therapist / Director of Patient Care Services (DPCS)

Position Summary:

The Certified Occupational Therapy Assistant (COTA) provides skilled therapy services to home health patients under the supervision of a licensed Occupational Therapist. The COTA implements established care plans, assists patients in regaining functional independence, and documents all services accurately in the agency's EMR system in compliance with Medicare and agency standards.

Responsibilities:

  • Provide occupational therapy treatments and interventions as directed by the supervising Occupational Therapist (OT) and the Plan of Care.
  • Observe, document, and report patient responses, progress, and changes in condition to the supervising OT and interdisciplinary team.
  • Educate patients and caregivers on home exercise programs, adaptive equipment use, and safe performance of daily living activities.
  • Accurately document all visits, interventions, and patient education in the EMR system within agency timeframes.
  • Collaborate with nursing, PT, MSW, and other disciplines to ensure coordinated, goal-oriented care.
  • Participate in case conferences and assist in revising the Plan of Care as appropriate.
  • Maintain compliance with Medicare, state regulations, and agency documentation standards.
  • Promote patient safety and functional independence in the home environment.

Qualifications:

  • Education: Graduate of an accredited Occupational Therapy Assistant program.
  • Licensure: Active and unrestricted California Certified Occupational Therapy Assistant (COTA/L) license.
  • Experience: At least 1 year of clinical experience preferred; home health experience highly desirable.
  • Skills:
  • Proficiency in EMR documentation (Axxess, HomeCare HomeBase, Kinnser, or similar).
  • Strong communication and time-management skills.
  • Ability to work independently and manage caseload efficiently.
  • Requirements:
  • Reliable transportation and valid CA driver's license.
  • Current CPR/BLS certification.
  • Proof of professional liability insurance (if applicable).

Compensation & Schedule:

  • $65 per completed home health visit (includes documentation).
  • Flexible per diem schedule based on patient needs and staff availability.
