Overview
The Nurse Practitioner (NP) will provide comprehensive clinical evaluation, education and health management for patients that present for routine, preventive, acute and chronic disease health care services, including onsite visits, telehealth and home visits. The nurse practitioner will coordinate all the services necessary for health promotion, maintenance, rehabilitation and the prevention of disease and disability.
The NP will be responsible for direct patient care and for other administrative duties as designated by the medical director.
Responsibilities, Methodology & Outcomes
- Assesses and interprets medical and psycho-social histories; performs physical examinations ( identifies problems; formulates a plan of treatment; provides education and referral (as indicated); and evaluates the effectiveness of the interventions prescribed.
- Assess patient needs for additional services such as case management, legal services, and specialty care. Refer and assist patients to access additional supportive services.
- Performs and orders laboratory tests and interprets the results as part of the health management. Reports communicable disease in a timely manner to the appropriate health department as per protocol;
- Conducts health teaching and counseling during the medical encounter, as appropriate, enabling the patient to make informed choices about health and treatment options; Educate patients on diseases, primary care issues, and adherence strategies to medications prescribed;
- Assess patient risk behaviors and educate patients in communicable disease risks and prevention
- Prescribes medications.
- Follows established treatment guidelines as indicated by C3 protocols;
- Completes the client's clinical record documentation in a timely manner specific to the applicable policies and procedures of C3
- Provides monitoring and continuity of care between visits.
- Maintains professionalism and adheres to client confidentiality per HIPPA regulations
