About Sunrise: Since 2015, Sunrise Homeless Navigation Center is the largest provider of homeless services in Travis County We offer pathways to housing for people experiencing homelessness through low-barrier access to wraparound services by providing innovative, trauma-informed, and person-centered programming that engages our communities and leads system-wide transformation. Through the Sunrise Hub day center, Sunrise Hotline call center, Sunrise Mobile outreach and shelter services, and Sunrise Housing program we serve more than 8,000 people experiencing homelessness each year.

Job Title: Hub Assessor

Position Status:, PT

Location: Day Center Hub

Compensation: $25 per hour: determined by Sunrise's structured compensation framework; PTO, Health/Retirement Reimbursement up to $1,000 per month

Reports To: Hub Program Manager

FLSA Classification: Non-exempt

Position Summary: The Hub Assessor at Sunrise Homeless Navigation Center serves as a vital link in our mission to provide comprehensive support to individuals experiencing homelessness or those at risk. You will conduct Coordinated Housing Assessments, provide referrals, and ensure data accuracy within the Homeless Management Information System (HMIS). Your work is essential to connecting clients with the services they need to move towards stable housing.

Position Qualifications:

  • Current or previous HMIS licensure preferred.
  • Bilingual (Spanish) preferred.
  • Bachelor's degree required. Applicants with the ability to supervise Social Work interns will be given preference.
  • Experience successfully navigating information systems and tech
  • Stable internet access required for Hotline assessments

Position Responsibilities:

  • Complete coordinated assessments and refer clients for Wayfinder prevention, diversions/rapid exits in person at the Hub.
  • Facilitate connections to Sunrise programs and case managers.
  • Actively listen to the client's stated needs and make appropriate resource referrals.
  • Consistently and accurately enter information into the HMIS database.
  • Assist with the collection and maintenance of data for statistical and programming purposes.
  • Navigate and utilize digital information on a daily basis.
  • Collaborate regularly with different Sunrise teams and outside service providers.
  • Occasionally attend in-person meetings.
  • Maintain a safe work environment and confidentiality at all times.
  • Be proactive, creative, and flexible in determining, evaluating, researching, and resolving issues
  • Organize and prioritize multiple activities to meet all external and internal deadlines
  • Maintain a professional demeanor that reflects positively on the agency.
  • Demonstrate respect and courtesy toward others.
  • Able to thrive in a work environment emphasizing teamwork and collaboration.
  • Respond promptly in all aspects of communication.
  • Work with minimum supervision.
  • Demonstrate strong boundaries in decision-making and safety planning.
  • Perform other duties as assigned.

Position Considerations:

  • Work is conducted in an open, communal environment with frequent public interaction and limited private office space. This position provides services to individuals at risk or are currency experiencing homelessness.
  • Exposure to individuals in crisis, including those experiencing mental health challenges, substance use, or medical conditions.
  • Due to licensing restrictions, job duties may vary for the first few months while licensure is acquired.
