Salary Range: $75,000 – $85,000 annually (DOE)Mileage reimbursement Full benefits package
What You'll Do:Manage and coordinate the care of patients in their homesPerform initial assessments, recertifications, develop care plans, and provide skilled nursing careCollaborate with physicians, therapists, other community partners, and caregivers to ensure optimal outcomesEducate patients and families on managing conditions and medicationsDocument accurately and timely in the EMRParticipate in case conferences and team meetingsSN visits as needed
