Job Description

The Director of Patient Services (DPS) oversees the clinical operations of the home care agency across multiple branches, ensuring high-quality patient care and compliance with healthcare regulations and standards. The DON is responsible for managing nursing staff, developing and implementing patient care policies, and collaborating with other healthcare professionals to provide comprehensive home care services across all branches under the same license

Responsibilities:

    1. Oversee and manage the nursing staff across all branches, including training, evaluation, and scheduling
    1. Develop and implement policies and procedures to ensure compliance with regulatory requirements and industry best practices
    1. Foster a positive and collaborative work environment, promoting professional development and continuing education opportunities for nursing staff and clinical staff
    1. Ensure high-quality patient care through effective assessment, planning, implementation, and evaluation of nursing services
    1. Develop individualized care plans in collaboration with patients, families, and healthcare providers
    1. Monitor patient outcomes and address any issues or concerns promptly
    1. Ensure all branches comply with federal, state, and local regulations, as well as accreditation standards
    1. Conduct regular audits and quality assurance reviews at each branch to identify areas for improvement and implement corrective actions as needed
    1. Maintain accurate and up-to-date patient records and documentation across all branches.
    1. Collaborate with other healthcare professionals, including physicians, therapists, and social workers, to coordinate comprehensive patient care
    1. Serve as a liaison between patients, families, and the healthcare team to ensure effective communication and patient satisfaction
    1. Participate in interdisciplinary team meetings and case conferences across all branches.
    1. Assist in the development and management of the nursing budget for all branches, ensuring cost effective delivery of care
    1. Contribute to the strategic planning and development of the home care agency's services and programs across all branches
    1. Identify opportunities for growth and improvement in patient care and services
    1. Regularly visit each branch to oversee operations and ensure consistent application of policies and procedures
    1. Provide support and guidance to branch managers and nursing staff to address any challenges and ensure a high standard of care
    1. Conduct state surveys for all branches and create plan of correction
    1. Hold monthly and quarterly Quality Assurance meetings
    1. Investigate incidents, complaints and grievances in order to action accordingly to our policies and to the wellbeing of our clients and team members
    1. Coordinate with Administrator planning activities to develop administrative policies and practices for the Agency
    1. Perform and prepares probationary and annual performance evaluations for professional staff

COMPETENCIES AND RESOURCES

1. Educational Level Required

  • Nursing Degree
  • Certification in home health care or nursing administration a plus not required

2. Qualifications and Requirements

  • Microsoft Office Suite experience
  • 5 years of as a registered nurse experience
  • 2 years in a supervisory or management role preferably in home care or community health setting

3. Skills

  • Strong leadership and management skills, excellent communication and interpersonal skills, Ability to Work Without Supervision, Planning and Organization, Teamwork, Dependability, Responsibility, Problem-solving
