About the Role
Avalanche Care is seeking a detail-oriented Registered Nurse to deliver high-quality home care through comprehensive assessments/reassessments, aide supervisions, and care-plan development—while supporting HHA orientation/in-servicing and documentation in office. You'll collaborate daily with our interdisciplinary team to keep plans of care accurate, timely, and compliant.
What You'll Do
- Conduct clinical visits: assessments, reassessments, and aide supervisions in patients' homes, using proper bag technique and infection-control best practices.
- Train & supervise HHAs: orient HHAs to the plan of care during in-home visits and remotely; assist with office-based orientation & in-services.
- Plan of care & documentation: establish/maintain individualized care plans; complete accurate, timely progress notes and visit records in the EMR.
- Team coordination: communicate promptly with Coordinators and Nurse QA Manager on visit schedules, missed visits, changes, and potential discharges.
- Quality & compliance: follow Avalanche Care policies, payer rules, and NYSDOH standards; participate in ongoing education and skills updates.
Qualifications
- Valid New York RN license (required).
- 1+ year of home healthcare (or related) experience.
- Strong clinical assessment skills; solid infection-control knowledge.
- Excellent communication and interpersonal skills; able to work independently and on a team.
- EMR proficiency (preferred).
- Reliable transportation and valid driver's license.
