  • upervisory Oversight : Provide guidance and support to Personal Care Attendants within the Primary Home Care department.

  • ervice Monitoring : Ensure that client services are executed according to task assignments and adhere to agency policies.

  • nsite Assessments : Periodically assess the quality of services provided at clients' homes.
    C
  • lient Communication : Engage with clients and their families to ensure satisfaction with services.
    I
  • ssue Reporting : Report any concerns or issues to the appropriate agency staff for resolution.

  • raining and Orientation : Orient Personal Care Attendants to new client assignments, explaining tasks and schedules while assessing their competency.
    P
  • erformance Evaluation : Evaluate the job performance of attendants and provide constructive feedback.

  • ocumentation and Compliance : Complete necessary documentation and supervisory visit reports, including all relevant information regarding client services.

  • lient Coordination : Notify family members or physicians of significant changes in a client's health or safety and coordinate care with agency programs.

ualifications/Experience:

  • ducation : Minimum of a high school diploma or G.E.D.; completion of two years at an accredited college or university preferred (substitutable with relevant supervisory experience).

  • xperience : 2 years of supervisory experience in a healthcare facility, home care agency, or community agency is required; 2 years of experience in home and community care is preferred.

  • anguage Skills : Bilingual proficiency is required.

  • icenses : Valid State Driver's License with a good driving record.
