Shodair Children's Hospital is seeking a dedicated and compassionate Resource Patient Care Coordinator to play a key role in supporting children and adolescents facing mental health challenges. This position ensures each patient receives a coordinated, individualized, and compassionate care experience from admission through discharge.

Key Responsibilities:

  • Complete patient intake procedures and initiate treatment plans upon admission.
  • Build strong, therapeutic relationships with patients, families, and interdisciplinary team members.
  • Coordinate and monitor care through treatment planning, crisis planning, and regular documentation.
  • Serve as liaison with community resources, schools, agencies, and external healthcare providers.
  • Conduct safety checks, assess belongings, and manage EPIC registration and documentation.
  • Facilitate discharge planning, aftercare coordination, and patient advocacy.
  • Participate in team meetings, patient rounds, and therapeutic unit activities.
  • Ensure culturally sensitive care and maintain HIPAA compliance.

Requirements:

  • Minimum 1 year of experience working with individuals with mental illness (required)
  • Bachelor's degree in a human services field (strongly preferred)
  • 2 years of experience in case management or healthcare/social services coordination (preferred)
  • Experience in an inpatient behavioral health setting (preferred)
  • Post-secondary education in human services (preferred)
