Resource Patient Care Coordinator

Shodair Children's Hospital is seeking a dedicated and compassionate Resource Patient Care Coordinator to play a key role in supporting children and adolescents facing mental health challenges. This position ensures each patient receives a coordinated, individualized, and compassionate care experience from admission through discharge.
Key Responsibilities:
- Complete patient intake procedures and initiate treatment plans upon admission.
- Build strong, therapeutic relationships with patients, families, and interdisciplinary team members.
- Coordinate and monitor care through treatment planning, crisis planning, and regular documentation.
- Serve as liaison with community resources, schools, agencies, and external healthcare providers.
- Conduct safety checks, assess belongings, and manage EPIC registration and documentation.
- Facilitate discharge planning, aftercare coordination, and patient advocacy.
- Participate in team meetings, patient rounds, and therapeutic unit activities.
- Ensure culturally sensitive care and maintain HIPAA compliance.

- Proficiency with electronic medical records (EPIC experience a plus)

Requirements:
- Minimum 1 year of experience working with individuals with mental illness (required)
- Bachelor's degree in a human services field (strongly preferred)
- 2 years of experience in case management or healthcare/social services coordination (preferred)
- Experience in an inpatient behavioral health setting (preferred)
- Post-secondary education in human services (preferred)
