The Community Healthcare Worker assists the Integrated Care Team (Primary Care Provider, Nurse/Medical Assistant, Nurse Navigator, Case Management) by proactively coordinating care for patients to facilitate the achievement of quality and cost effective health outcomes.

Required Qualifications:

Education and Certifications:

  • High school graduate
  • Minimum of one (1) year experience working in a related role within the healthcare or social services setting
  • Must achieve certification as a Community Health Worker in the State of New Mexico within one (1) year from hire
  • Must have reliable transportation to make home visits as needed within San Juan County

Preferred Qualifications:

  • Associate or higher degree in a related field of study
  • Five (5) or more years of experience working in a related role within the healthcare or social services setting
  • Certified Community Health Worker in the State of New Mexico
  • Multilingual – English, Spanish and/or Navajo

Duties and Responsibilities:

  • Ability to provide excellent patient care with families based on mutual trust and respect
  • Articulate; good communication skills with patients in person, telephone, virtual and/or e-mail
  • Good problem-solving skills
  • Knowledge of computer operation
  • Excellent customer service skills
  • Professional and caring manner with patients, clients, and families
  • Self-Starter with strong verbal and written communication skills
  • Experience with Motivational Interviewing strongly preferred
  • Bilingual (English/Spanish) or (English/Navajo) preferred
  • Knowledge of local community resources
  • Experience with Electronic Health Records
  • Supportive of and responsive to the provider needs and acts in an organized and adaptable manner
  • Maintains confidentiality and privacy of the patient
  • Punctual and dependable
  • Strives to consistently achieve excellence in all assignments, tasks, and goals
  • Provides personalized care and assistance to patients, following the SJRMC Mission, Vision and Philosophy
  • Documents pertinent information in a legible manner, and in accordance with clinic and regulatory standards

  • Assumes responsibility and assists in anticipating, identifying, and solving problems
  • Collaborates with the clerical and clinical staff to provide excellent patient care and customer service
