The MDS (minimum data set) coordinator, also known as resident assessment coordinators and nurse assessment coordinators, MDS coordinators:

  • Reports to facility Administrator, partners with DON
  • Coordinates, conducts, and completes the MDS, resident care plans and conferences on the computerized system.
  • · Assures and monitors that MDS are completed
  • · Ensures MDS are completed and electronically submitted per regulation.
  • Ensures resident care plans reflect the cares being provided.
  • · Participates in family/resident care plan conferences.
  • · Assures effective communication with other shifts and departments as it relates to medical record documentation and completion of assigned MDS sections..
  • · Assures timeliness of physicians' visits.
  • · Notifies physician and family of changes in resident's condition.
  • · Encourages rehabilitative techniques in order to restore residents' independence.
  • · Ensures MDS are completed and electronically submitted per State regulations.
  • · Attends in-service training and education sessions, as assigned.
  • · Possess basic computer skills, with strong knowledge of MDS computer software.
  • Must be able to work independently as well as with a team
  • Performs specific work duties and responsibilities assigned by supervisor.

Education and/or Experience:

  • RN license
  • 1 year experience preferred, but will train the right candidate
  • Basic knowledge of Skilled nursing and long term care facilities
  • 1 year of supervisory experience
  • Attention to detail is an absolute must

Salary: Depending on Experience

Job Type: Full-time

Pay: From $45.00 per hour
