Position Summary:

We are seeking a reliable, organized, and empathetic Nursing Assistant (NA or CNA) to support our physicians and nurse practitioners in the field. You'll serve as a vital point of connection between our providers and the patients we care for—ensuring seamless visits and respectful interactions.

Responsibilities:

  • Travel to patient locations (private homes, assisted living, etc.) alongside or in coordination with providers
  • Take vital signs, assist with patient assessments, and record basic health information
  • Support providers with mobility, equipment setup, and documentation
  • Assist with toenail trimmings and ear cleanings (training will be provided)
  • Assist with light patient care, such as positioning or toileting, when needed
  • Help with appointment coordination, reminders, and follow-up tasks
  • Communicate observations clearly and promptly to the medical team
  • Maintain patient privacy and uphold HIPAA compliance at all times

Requirements:

  • No CNA Certification required, we will train the right canidate
  • Reliable personal transportation with valid driver's license and insurance
  • Clean driving record
  • CPR certification (BLS preferred), we will help obtain this if needed
  • Excellent communication and interpersonal skills
  • Comfortable working independently in varied environments
  • Experience with older adults or home health preferred
  • Tech-savvy (basic proficiency in mobile apps and EMRs)
