Vita Blue Home Care provides high-quality, client-focused home care services throughout the region. We are seeking an experienced and results-driven home care sales professional who understands referral development and can generate measurable growth through strong community relationships.
Position Overview:
This is a salary-based hybrid role. The Community Liaison is expected to:
- Spend most of the week in the field meeting referral partners, conducting outreach, and generating new business, and
- Be in the office at least two days per week for reporting, strategy development, and coordination with internal teams
Required Qualifications:
- Proven track record in home care sales is required
- Experience cultivating relationships with healthcare referral sources (SNFs, hospitals, rehabilitation centers, social workers, etc.)
- Ability to independently manage territory planning, scheduling, and follow-up
- Strong communication, presentation, and closing skills
- Valid driver's license and reliable transportation
- Bilingual preferred
Key Responsibilities:
- Develop and strengthen referral relationships with hospitals, skilled nursing facilities, assisted living communities, and community organizations
- Conduct regular in-person visits and professional presentations
- Generate qualified leads and convert referrals into active clients
- Maintain detailed activity logs and reporting
- Work closely with internal teams to ensure strong client onboarding and service continuity
- Meet monthly and quarterly sales goals
Compensation & Benefits:
- $55,000 base salary
- $500 commission for each new client who remains active for 30 days
