Wound Care Intake Coordinator and Patient Navigator

Job Summary
This position ensures smooth coordination of wound-care referrals, timely intake processing, accurate documentation, and compassionate guidance to patients and families throughout their wound-care journey.
Duties
- Receive, review, and process all wound-care referrals from hospitals, SNFs, clinics, or physician offices.
- Verify patient demographics, insurance eligibility, and clinical documentation (orders, wound photos, H&P, etc.).
- Communicate promptly with referring physicians to obtain missing documents or clarifications.
- Enter patient information into the EMR and initiate the wound-care episode within required timelines.
- Coordinate scheduling of wound-care visits with RNs, LVNs, or wound specialists.
- Ensure consent forms, orders, and treatment plans are properly signed and filed before the first visit.
- Serve as the main contact for patients and families from referral through discharge.
- Educate patients and caregivers about wound-care services, visit expectations, and plan of care.
- Follow up after hospital discharges to ensure continuity of wound treatment.
- Provide reminders and support for follow-up appointments, dressing changes, and supply deliveries.
- Monitor patient satisfaction and resolve communication or service concerns quickly.
- Collaborate closely with wound-care nurses, NPs, and MDs to ensure timely treatment orders and care delivery.
- Track wound progress reports, weekly photos, and clinical documentation for quality monitoring.
- Maintain up-to-date wound formulary and supply inventory lists.
- Ensure wound orders align with payer guidelines and medical necessity requirements.
- Participate in interdisciplinary team meetings to discuss patient progress and barriers to healing.
- Support scheduling optimization to minimize gaps and delays in care delivery.
- Maintain accurate logs of all referrals, admissions, and pending authorizations.
- Document all communications with patients, families, and providers in the EMR.
- Generate daily or weekly intake activity reports for leadership review.
- Assist in coordinating authorizations and re-certifications for ongoing wound care.
- Ensure compliance with HIPAA, Medicare CoPs, and internal policies.
- Verify that all intake packets, orders, and wound photos are complete and signed before billing.
- Monitor metrics such as referral conversion rate, admission turnaround time, and patient satisfaction.
- Participate in chart audits, staff training, and quality improvement initiatives.
Qualifications
- LVN, RN, or equivalent clinical background preferred.
- Experience in wound care, home health, or hospice intake coordination strongly preferred.
- Strong communication, multitasking, and EMR documentation skills.
- Knowledge of Medicare/Medicaid and commercial insurance processes.
- Compassionate, patient-centered approach with professional telephone etiquette.
- Provide feedback to marketing and outreach teams on referral patterns and growth opportunities.
- Educate community partners about available wound-care programs and services.
- Attend networking meetings or case conferences as needed to represent the agency's wound-care program.
