Job Summary
The Social Care Navigator serves as a vital link between healthcare providers and the community, focusing on improving health outcomes and access to services. The SCN will engage with individuals and families to educate them about health resources, provide support for managing chronic conditions, and promote healthy behaviors. This role requires a compassionate individual who is skilled in motivational interviewing and has a strong understanding of community health issues.
Social Care Navigator (Full-Time 40 Hours/Week) :Job Description:
- Conduct screenings for Health-Related Social Needs (HRSN) using standardized tools telephonically.
- Assess client needs and eligibility for available community resources or public programs.
- Develop personalized care plans and issue referrals using navigation platforms.
- Follow up with clients and referral partners to ensure services are accessed (closed-loop referrals).
- Collaborate with pharmacy team members, including pharmacists and technicians, to coordinate care.
- Participate in staff meetings, trainings, and community engagement events as needed.
Position Summary:
The Social Care Navigator will play a vital role in addressing patients' non-medical needs—such as food insecurity, housing instability, transportation barriers, and more. This person will serve as a trusted point of contact, conducting social needs screenings, making referrals to community-based services, and tracking outcomes to ensure support is provided.
This is a full-time position based on-site with community outreach, working closely with pharmacy staff, local clinics, and community organizations.
Duties
- Conduct outreach to engage community members and assess their health needs.
- Educate individuals and families about available health resources, including Medicare and addiction counseling services.
- Facilitate access to healthcare services by assisting with appointment scheduling and transportation arrangements.
- Collect data on community health trends and individual patient outcomes to inform program development.
- Provide support through motivational interviewing techniques to encourage behavior change and adherence to treatment plans.
- Deliver public speaking presentations on health topics to community groups, schools, and organizations.
- Collaborate with social workers, nurses, and other healthcare professionals to coordinate care for clients.
- Maintain accurate records of interactions with clients and follow up as necessary.
Requirements
- Proven experience in social work or a related field is preferred.
- Strong knowledge of medical terminology and healthcare systems.
- Excellent communication skills, both verbal and written, with experience in public speaking.
- Familiarity with data collection methods for assessing community health needs.
- Ability to work independently as well as part of a multidisciplinary team.
- A background in addiction counseling is a plus but not required.
- A genuine passion for helping others improve their health and well-being.
- Must be able to demonstrate empathy, patience, and cultural competence when working with diverse populations.
