Job Overview
The Behavioral Health Case Manager creates, manages, and develops comprehensive behavioral health and substance use disorder plans through a series of case management and social work interventions. Triage assessments are conducted to determine appropriate levels of care needed and refer patients appropriately to providers and programs. Role will focus on transitions from In Patient Care to Outpatient services. It will also focus on outpatient utilization review and case management including the coordination of care with the primary care physician and nursing staff to assure patient compliance with appointments and treatment plan. Also of significance will be regular communication with the patient and the patient's family and caregivers to ensure patient adherence to their behavioral health treatment plans.
Duties
- Develop and implement behavioral health programs and initiatives
- Coordinate discharge planning for patients with mental health needs
- Collaborate with interdisciplinary teams to provide comprehensive care
- Conduct mental health counseling and addiction counseling as needed
- Manage inpatient behavioral health services
- Work closely with emergency medicine staff to address behavioral health crises
- Utilize managed care principles to optimize patient outcomes
Experience
- Must have current California LVN, LPT or RN license
2+ year experience in Behavioral Health Utilization Review or Case Management
Knowledge of Behavioral Health industry practices with preferred emphasis on managed behavioral care
Job Type: Full-time
Pay: $ $38.00 per hour
