In My Home Hospice Care is seeking a full-time LPN (Monday - Friday 8am-5pm) Must be willing to drive to different areas in the Phoenix Metro Area primary East Valley.

The LPN works under the direction of a Registered Nurse and provides nursing and palliative care to assigned hospice patients.  Must possess a sympathetic attitude toward caring for hospice patients and families and must have positive communication skills interaction with other members of the hospice team.  Participates in coordination of hospice services, appropriately reporting the identified needs to the interdisciplinary group.  Provides appropriate pain/symptom management/evaluates patient's response to treatments/medications.  Performs other duties as assigned by the registered nurse.

Position Qualifications
  • Graduate of an accredited practical nurse or vocational nursing program.
  • Community health/hospice is preferred but willing to train the right candidate.
  • Currently licensed as an LPN/LVN in the State of Arizona.
  • Complies with accepted professional standards and practice.
  • Demonstrates good verbal and written communication, and organization skills.
  • Must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and is in good working order.
  • Possesses and maintains current CPR certification.

Compensation: $62, $68,500.00 per year

We started serving in 2004 as Supreme Home Care, specializing in wound care and other in-home services that require skilled nursing. We were asked to provide hospice care as well and have finally begun doing so. We have always focused on excellent in-home care, so this was a natural step. After going through the experience of needing hospice care for both my mother and mother-in-law two years ago, I know what the most important things are for hospice care.
  • First, both moms wanted their care to be in their homes as long as possible. That meant a lot of supports were needed to be put in place.
  • Second, they wanted to be treated with love and dignity by those supports.
  • Third, they wanted as little pain and suffering as possible.
We have now included doing business as "In My Home Hospice Care" because that has always been our number one focus, providing "in-home" care. We want you to feel comfortable while receiving the best care in town, and in this regard, we spare no expense.
