HCBS/Children's Waiver Coordinator

We are seeking a detail-oriented and compassionate individual to fulfill the role of HCBS/Children's Waiver Case Coordinator. The Case Coordinator will be responsible for coordinating services for clients, maintaining case records, and facilitating communication between clients, families, and service providers. Responsibilities:
· Coordinate and facilitate services for clients.
· Maintain accurate and updated case records, ensuring completeness and confidentiality.
· Communicate effectively with clients, families, and service providers to ensure that the
needs of clients are met.
· Assist in developing and implementing case management plans in collaboration with
program director, clients and other stakeholders.
· Conduct fieldwork to support families.
· Provide support and guidance to clients and their families throughout the case management
process
· Collaborate with other professionals, such as social workers and healthcare providers, to
ensure comprehensive care for clients.
· Attend all training and meetings offered by the Department of Health (DOH) regarding
HCBS/Children's Waiver.
· Attend meetings with team on an annual, semi, and when needed
· Collaborate with Director of Program Development to become approved to provide more
service under HCBS/Children's Waiver.
· Responsible for intake, hiring, training and supervision of Waiver Service providers
Skills:
·Bachelor's degree in social work, psychology, or a related field.
·Driver's License (NY State in Good Standing)
· Proven experience in HCBS/Children's Waiver (a must), case management, social services, or a related field may be preferred.
·Strong organizational and time management skills.
· Excellent written and verbal communication skills.
· Empathy and compassion for the clients and families being served.
· Ability to work effectively both independently and as part of a team.
· Proficiency in relevant computer applications and software.
Job Type: Full-time
Pay: $60, $65,000.00 per year
