Schedule: Weekends - Saturday & Sunday 8:00 AM - 5:00 PM
Under the general administrative direction of the reporting manager, the E-Referral Specialist will receive and respond to community, physician, patient and family request for home health. This position will record and process referral information as well as document in patient information database. The E-Referral Specialist will schedule registration and admission visits, prepare all required paperwork for admission as well as act as a liaison between patients, families, referral sources, physicians and admission staff. They will also assist in distribution of admission information and report to primary teams and physicians.
Responsibilities- Perform required data entry accurately at the times specified by the department.
- Patient information and forms are managed and forwarded to the appropriate department per procedure.
- Monitor all assigned portals for new referrals, messages, bookings, and updates.
- Work with the intake department and local branches to ensure communication of all booked referrals, referral sources messages and updated documents coming through the portal and to minimize any delays in care of our patients.
- Associate's degree preferred; High School Diploma required.
- 1-2 years of experience in related healthcare position.
- Proficient in MS Office Suite (MS Word, TEAMs, Excel, Outlook)
- Previous EMR portal experience preferred.

- Uses own knowledge and experience base and other resources as necessary to make logical decisions and solve problems.
- Develop and maintain professional relationships with peers, sales, operations, and intake teams.
- Maintain competence in working with a multitude of applications such as; ZipTools, Teams, Outlook, All portals, HCHB and Insurance verification sites.
- Excellent interpersonal communication and presentation skills required
- Excellent selling, organization, problem-solving skills, and the ability to appropriately represent the Company service capabilities to the targeted referral source audience as required
- Ability to prioritize workload and ensures that all projects are completed by deadlines.
- Ability to travel within assigned territory and to sales meetings as required and may be required to travel to another agency or training location periodically
