RN - Community Clinics, Per Diem

Under the direction of the Community Clinics & Infection Control Manager provides skilled professional nursing care to clients in the community and VNHH staff in accordance with the policies and procedures of the Visiting Nurse Home & Hospice. Gives care in a manner which promotes quality, continuity and safety for the client and the community.
Functions:
Provides professional nursing care utilizing all elements of the nursing process.
Recognizes and utilizes opportunities for health care teaching with patient and/or family members.
Participates in orientation program for new nurses, other professional staff, certified nursing assistants and student nurses.
Completes necessary clinic forms and maintains daily time sheets. Refers clients for follow-up and medical care as needed.
Follows policies and practices of the Visiting Nurse Home & Hospice.
Accepts assignments and performs duties within the limits of preparation and experience.
Accepts responsibility to maintain and upgrade professional knowledge and skills through attendance and participation in continuing education and in-service programs.
Applies measures to prevent and control the spread of communicable diseases as required by Rhode Island Department of Health. Complies with the agency's Infection Control Program.
Attends and participates in interdisciplinary team meetings.
Education and Experience:
Graduation from a school of nursing accredited by NLN:
From a baccalaureate or masters degree program in nursing.
From a state approved diploma or associate degree program in nursing.
Nurses without a baccalaureate degree must have four years experience in nursing. Staff are encouraged to pursue their education toward a baccalaureate degree in nursing.
Registered as a professional nurse in Rhode Island.
Membership in professional nursing organization is desirable.
Interest in keeping abreast of trends and developments in nursing field.
Current certification in CPR.
Qualifications:
Comprehensive knowledge of current nursing practice.
Ability to make meaningful observations and to complete concise, accurate reports.
Thorough knowledge of and ability to interpret Agency programs and policies to patient/family, and public.
Ability to relate objectively with co-workers, patient/family, other professionals in the agency and in other agencies and the public.
Ability to recognize needs and concerns of people and to deal constructively with them.
Knowledge of and ability to use community resources.
Ability to study and evaluate own job performance and to plan and carry out program for continuing professional growth.
Maintains current R. N. licensure in Rhode Island and obtains necessary CEU's to maintain licensure.
