At OWL Early Intervention Program, our mission is to empower infants and toddlers with developmental delays through high-quality, family-centered services. As we continue to grow, we are seeking a dedicated and compassionate Early Intervention Case Manager to join our team.

Position: Early Intervention Case Manager

Schedule: Full-Time

Location: 1016 Washington Ave., Bronx, NY 10456

Key Responsibilities

  • Serve as the primary point of contact for families enrolled in the Early Intervention Program.
  • Coordinate and oversee services, including IFSP implementation and ongoing reviews.
  • Review session notes to ensure providers are delivering services according to the mandated frequency and duration.
  • Maintain consistent communication with families, providers, and multidisciplinary team members.
  • Complete and monitor First Attend Dates, GAP documentation, and all service delivery requirements.
  • Ensure timely initiation of services and full compliance with all EI regulations.
  • Manage a caseload effectively, maintaining accurate documentation, service orders, and authorizations.
  • Track provider service delivery and follow up on delays or concerns.
  • Advocate for families and support them in accessing appropriate resources and services.

Qualifications

  • Bachelor's or Master's degree in Social Work, Psychology, Early Childhood Education, Human Services, or a related field (preferred).
  • Early Intervention and/or case management experience strongly preferred.
  • Minimum 6 months of experience using EIHub required.
  • Experience with ProviderSoft is preferred.
  • Strong organizational skills and exceptional attention to detail.
  • Compassionate, family-centered communication and interpersonal skills.
  • Ability to collaborate effectively with a multidisciplinary team.
  • Knowledge of NYC/NYS EI policies is a plus (training provided).
