JOB SUMMARY:

The Service Coordinator Over Three (SCOT) supports families whose children (ages 3–6) qualify for services through the South Carolina Department of Disabilities and Special Needs (SCDDSN). In this role, you'll guide the development and implementation of Family Service Plans (FSPs), working closely with families to coordinate services, monitor progress, and connect them with resources. Services are provided in natural environments (e.g., home, preschool, community), and flexibility is key—some evening or weekend visits may be needed. Services are designed to be individualized, child-centered, family-guided, community-based, and culturally responsive. Timely documentation and coordination are essential, as required by SCDDSN guidelines. Home visits are a key part of the role, so flexibility in scheduling is necessary to meet the needs of families—including occasional evening or weekend appointments. Service Coordinator Over Three Responsibilities:

  • Caseload Management:
  • Provide services to families and serve as a liaison to assist them in accessing and navigating services and supports outlined in the Family Service Plan (FSP)
  • Family Partnership:
  • Establishes and maintains professional relationships with families based on trust and respect and provides supportive assistance to families.
  • Works with families and caregivers to develop functional FSP goals or outcomes.
  • Provides families with information on current trends and practices related to the needs of their child and assists families in establishing their role as an advocate for their child.
  • Provides linkage, referral, advocacy, service planning, and crisis support as needed.
  • IFSP Development & Monitoring:
  • Monitors FSPs in accordance with SCDDSN policies and procedures.
  • Schedules, develops, implements, monitors, and evaluates the FSP for each family on caseload, ensuring that appropriate procedures and timelines are followed as established by policies and procedures established by SCDDSN and BabyNet.
  • Coordination of Services:
  • Coordinates services provided to the client/family by on-going information sharing through participation in interagency meetings, telephone calls and/or face to face contact with other FSP team members or outside agencies.
  • Assists in the child and family gaining access to health care, community resources, services, and/or funding sources to help meet their needs.
  • Documentation:
  • Submits accurate and timely routine documentation (clinical service notes) as required by SCDDSN and KIDS policies and procedures.
  • Ensures client records are accurately maintained, organized, and kept up-to-date as required by DHHS and KIDS policies and procedures.

MINIMUM QUALIFICATIONS

A Bachelor's degree in one of the following areas:

  • Education: Early Childhood, Special Education, Early Childhood Special Education, or Elementary Education
  • Child/Human Development
  • Interdisciplinary Studies (with approval from BabyNet State Office): with a focus in Early Childhood, Special Education, Early Childhood Special Education, or Elementary Education
  • Child Life
  • Family Studies/Family and Consumer Sciences
  • Psychology
  • Public Health
  • Social Work
  • Sociology

And (REQUIRED): At least one year of documented experience working with infants and toddlers OR working with children ages birth to 5 years with disabilities.

Must submit an application to the Team for Early Childhood Solutions (TECS) office and meet requirements for the South Carolina Infant-Toddler Credential.

PREFERRED: At least one year of experience with South Carolina's BabyNet Early Intervention Program.
