About Program:
NYC's foster care system serves over 8,000 children and youth ages 0 to 23+ who are temporarily placed in foster or kinship homes due to instances of abuse or neglect that make their ability to stay with their birth families unsafe. It is the responsibility of the EFFC team to work with the children, birth families, and foster/kin families to keep children in care safe, while addressing educational, health, and mental health needs and nurturing their development and overall well-being. The ultimate goal of a successful EFFC program is to work with the family to achieve permanency for the child(ren). Permanency may be achieved through a child's return to the parent, a child's long-term stay with kin (KINGAP), or adoption.
JOB LOCATION: Brooklyn/Queens transportation hub, accessible by the "L" and "M" trains as well as several bus lines.
POSITION SUMMARY:
This will be a great career opportunity for individuals interested in working with children and families, learning and implementing evidence-based models within the foster care system, providing case management services, and working closely with ACS and the Family Court system.
RESPONSIBILITIES:
Implement evidence-based models with foster parents, birth parents/discharge resources, and youth. This includes running groups and supervising family visits using an evidence-based approach.
Advise/counsel/advocacy/case planning with parents and/or discharge resources and children.
Attend all court proceedings promptly with prepared court reports and/or permanency reports.
Maintain a caseload of 12 cases on average.
Attend weekly team meetings, consultations, and supervision.
Acts as liaison; accompanies and advocates for family in medical, drug treatment, and screening, school, and housing settings.
Conduct home visits to the homes of foster parents and birth parents/discharge resources.
Document reports, progress notes, FASP, and plan amendments via Connections within the required periods.
Coordinate services amongst agency/community clinicians, ACS, educational providers, collateral agencies, and coworkers.
Coordinate and facilitate family and siblings visitation meetings. Supervise as required by the service plan and the evidence-based model.
Participate in all trainings as assigned.
Any other duties as assigned.
As a group facilitator or co-facilitator of the KEEP or PTC groups, the case planner is responsible for the following:
Conducts weekly groups with parents (foster, kin, biological, and adoptive).
Complete pre–session preparation in a timely fashion.
Attends and is on time to weekly coaching sessions.
Submits recordings of sessions and completes required session forms in a timely fashion
Reviews KEEP/PTC feedback and practices strategies with colleagues.
In addition to the group facilitation or co-facilitation, case planners are responsible for the following casework functions:
Assess and monitor all interactions with the child and family for safety and risk.
Conduct required casework contacts with the child, parent, foster parents, and collateral contacts (schools, service providers, medical personnel, mental health practitioners, and substance abuse counselors).
Make assessment-based referrals as necessary to meet the needs of the child and/or parent.
Participate in Family Team Conferences, including but not limited to placement preservation, reunification/discharge, goal changes, and permanency planning conferences.
Complete progress notes and other required documentation.
Complete FASP and plan amendments.
Attend Family Court and attend all permanency hearings.
MINIMUM REQUIREMENTS
BA Degree required with some related experience.
Experience in child welfare/foster care preferred.
Interviewing and assessment skills are essential.
Able to manage multiple priorities simultaneously.
Excellent writing skills.
Must have strong engagement skills.
Organizational skills: structure is essential to clearly negotiate workload.
Knowledge of the Child Welfare System preferred.
Some knowledge of the cultures represented in the NYC foster care system preferred.
Knowledgeable with computers.
Bilingual preferred but not required.
