Key Responsibilities

  • Provide compassionate, high-quality medical care in patients' homes and care facilities
  • Develop individualized treatment plans based on comprehensive health assessments and team collaboration
  • Educate patients and families on conditions, treatments, and care plans with empathy and clarity
  • Respect each patient's goals, values, and preferences while guiding them toward the best possible outcomes
  • Partner with families, caregivers, and interdisciplinary teams to ensure coordinated and continuous care

Qualifications

  • Active Nurse Practitioner license
  • Minimum of 2 years' experience in primary care (more is highly preferred)
  • Excellent bedside manner with a strong foundation in clinical care
  • Exceptional communication skills and the ability to build trust with patients and families
  • Team-oriented, reliable, and compassionate approach to care
  • Commitment to professional growth and lifelong learning
  • Palliative care experience is a strong plus

Schedule & Flexibility

  • Flexible daytime schedule
  • No formal on-call requirements (occasional patient inquiries may occur)
