General Description:
- Record all occurrences and conduct quality investigations for OPWDD (and HR occurrences as needed and assigned) and prepare investigation reports for Executive Administration.
- Conduct all interviews with victims, witnesses, subject(s) of the investigation, family or advocates when necessary, and all other staff with knowledge of an incident.
- Write full investigation reports (OPWDD # 149) for all OPWDD/Justice Center investigations.
- Provide the Incident Review Committee (IRC) with timely completed investigation reports and be prepared to answer questions from the committee.
- Ensure completion of all recommendations pertaining to incidents.
- Maintain consistent contact with OPWDD Incident Management Unit for guidance and direction.
- Possess basic knowledge of the IRMA (Incident Report and Management Application) and WSIR (Web Submission of Investigative Reports) systems through OPWDD/Justice Center.
- Maintain and manage general records of the agency's internal incident report system.
- Record all Incident Review Hotline calls agency-wide.
- Conduct internal program audits to meet and exceed quality standards, OPWDD compliance, and audit expectations.
- Maintain and implement an effective Corporate Compliance Plan.
- Correspond with the Division of Quality Improvement on an as-needed basis.
- Possess knowledge of OMIG (Office of the Medicaid Inspector General) regulations.
Responsibilities Include:
- The Incident Review/Corporate Compliance/Quality Assurance Coordinator will work closely with and under supervision of the Executive Administration.
- The Incident Review/Corporate Compliance/Quality Assurance Coordinator will be responsible for advising subjects of investigations of their situation as required by OPWDD.
- Mandatory training will be required in the IRMA/WSIR program systems.
- Coordinate with Human Resources on an "as needed" basis for purposes of OPWDD and internal HR investigations.
- Ensure that all incidents are entered into the IRMA/WSIR system as required by OPWDD.
- Monitor the IRMA dashboard for new incidents that may have been reported to the Justice Center regarding On Your Mark, Inc.
- Ensure thorough investigation of all incidents and allegations of abuse/neglect to conclusion with findings.
- Track recommended actions delegated to responsible program leaders.
- Collaborate with Directors to monitor the completion of corrective actions and implementation of all recommendations resulting from Incident Review Committee meetings.
- Review all incident investigative reports conducted by others to ensure that a thorough investigation was conducted, and a thorough report was written.
- Ensure the reporting (submission through the completed process of incidents) to OPWDD and the Justice Center as required.
- Ensure the submission of Corrective Action Plans (CAP) to the Justice Center as required.
- Monitor and facilitate abuse and neglect investigations conducted by the Justice Center as needed.
- Compile the information gathered during investigations to analyze trends.
•Submit analysis of incident trends to the Incident Review Committee (IRC) and Board of Directors, as requested. - Interpret Justice Center/OPWDD requirements for and provide support to the Directors of On Your Mark, Inc.
- Ensure confidentiality of all investigative information as per OPWDD/HIPAA (Health Insurance Portability and Accountability Act) regulations.
- Ensure that Executive Administration is kept informed of any ongoing investigations.
- All written documents that are created or gathered as part of an investigation should be kept safe and secure and submitted with the completed investigation upon presentation to the IRC.
- Participate in IRC and Corporate Compliance Committees.
- Prepare monthly Incident Review Summary Reports for Program Directors and Board of Directors meetings.
- Present annually to Board of Directors regarding Incident Review status and trends.
- Must be available on call 24/7.
- Active involvement in staff training and development, to include New Employee Orientation.
- Generate trend reports for the Board of Directors and Executive Team.
- Attend OPWDD/IAC trainings pertaining to Incident Management.
- Conduct Incident Management training for all employees and the Board of Directors.
- Perform other duties as assigned.
- Report vehicle accident, including obtaining documents and corresponding with insurance company
Knowledge, Skills and Abilities Required:
- Familiarity with OPWDD/Justice Center IT (Information Technology) Systems and computer programs.
- Familiarity with HRIS (Human Resources Information System).
- Demonstrates above-average written and verbal communication skills.
- Knowledge of quality management techniques, procedures, principles, and evaluation methodologies.
- Displays a high level of initiative, effort, and commitment towards completing assignments efficiently and accurately.
- Proficiency in using written and computer skills to prepare investigative reports and demonstrate attention to detail.
- Ability to analyze and see trends in various types of occurrences/investigations and recommend possible alternative approaches.
- Effective interviewing/interpersonal skills.
- Demonstrates responsible, confidential, and professional behavior.
- Demonstrates analytical abilities to develop key written points from factual findings after examining all evidence and prepares investigative reports using complex details resulting from interview summaries.
- Listens carefully during interviews and responds appropriately.
- Must remain non-biased/discreet/non-threatening and ensure that interviews with all pertinent parties are conducted efficiently and respectfully.
- Excels in creative thinking, time management, and problem solving.
- Knowledge of Federal, State and Local regulations pertaining to Labor Laws.
Requirements:
- Bachelor's degree in social work, psychology, education, or closely related field, preferred.
- Five (5) years of experience in the field of Intellectual and/or Developmental Disabilities, preferred.
- Knowledge and understanding of Residential Habilitation, Day Habilitation, Community Habilitation, Employment Services and Respite Services programs.
- Knowledge of OPWDD/Justice Center regulations.
- Must have a valid driver's license, some travel may be required.
- Experience with professional meetings.
- Must complete the Inter-Agency Council (IAC) Investigative Training Class certification and renew certification every three years.
- Current active New York Notary Public certification a plus.
