We're looking for a Family Nurse Practitioner to provide community based primary and palliative care. The patients are in LTC facilities, homes, assisted living, independent living and personal care homes. The ideal candidate will possess an understanding of Internal Medicine and Geriatrics. A successful candidate will be comfortable working independently in a fast-paced environment and will possess strong communication skills.

Responsibilities:

  • Direct patient care.
  • Ensure all consults are completed timely.
  • Collect program data.
  • Monday - Friday 8a-5p
