Position Summary

The Licensed Practical Nurse (LPN) provides clinical oversight and direct nursing support for Host Home and PCA clients. Responsibilities include conducting nurse delegations in accordance with the Colorado Nurse Practice Act, medication monitoring and setup, training non-licensed caregivers, and ensuring high-quality care for members with intellectual and developmental disabilities.

Key Responsibilities

Clinical Oversight and Nurse Delegation

  • Complete assessments and determine appropriate nurse delegations per the Colorado Nurse Practice Act
  • Delegate and train caregivers on health-related tasks such as medication administration, G-tube care, trach suctioning, oxygen use, and injections
  • Maintain records of all delegations and ensure timely re-assessments
  • Ensure delegated caregivers are operating within their scope and competence

Medication Oversight

  • Conduct routine medication monitoring to ensure correct administration and documentation
  • Perform medication counts and verify accuracy of medication administration records (MARs)
  • Set up medications in pill planners or medication boxes as permitted under nurse delegation rules
  • Ensure medication storage and handling meet regulatory standards
  • Communicate with providers and pharmacies regarding changes in medication orders

Documentation and Monitoring

  • Complete visit notes, health summaries, and monitoring forms for each client
  • Track expiration of delegations, training, and other compliance documents
  • Identify and report health-related issues or medication discrepancies
  • Maintain thorough and timely documentation in electronic systems such as Therap

Training and Compliance

  • Provide training to caregivers on standard precautions, infection control, and emergency response
  • Educate providers on recognizing signs of health decline and when to report changes
  • Assist in developing corrective action plans when non-compliance is identified
  • Support audit readiness by ensuring clinical documentation is complete and current

Collaboration and Care Coordination

  • Communicate regularly with interdisciplinary teams, guardians, case managers, and physicians
  • Participate in service planning meetings and help advocate for appropriate medical supports
  • Respond to urgent medical issues or incidents and provide clinical guidance when needed

Required Qualifications

  • Active, unrestricted Colorado LPN license
  • At least one year of LPN experience in home health, IDD, or community-based settings
  • Knowledge of the Colorado Nurse Practice Act and applicable nurse delegation protocols
  • Valid driver's license, reliable transportation, and proof of insurance
  • Ability to pass all required background and registry checks
  • Strong organization, communication, and independent time-management skills
  • Proficiency in electronic documentation (Therap experience preferred)

Preferred Qualifications

  • Prior experience with medication setup and medication counts in residential or IDD settings
  • Familiarity with DD, SLS, or CES waivers in Colorado
  • Skilled in G-tube, trach, oxygen management, or insulin administration
  • Bilingual (Spanish/English) a plus

  • Hourly Rate: $30–$38 per hour (commensurate with experience)
