Licensed Practical Nurse (LPN) – Long-Term Home Health (LTHH)

Location: Aurora, Colorado

Schedule: Full-time / Part-time / Flexible shifts

We're looking for a dedicated Licensed Practical Nurse (LPN) to join our Long-Term Home Health (LTHH) team and make a difference in the lives of Colorado Medicaid clients.

What You'll Do

As an LPN in Long-Term Home Health, you'll deliver skilled nursing care under the supervision of an RN Case Manager and in accordance with each client's individualized plan of care. Your role is hands-on, meaningful, and deeply rewarding.

Responsibilities include:

Provide one-on-one skilled nursing care in the client's home.

Administer medications and treatments as ordered.

Perform wound care, tracheostomy and ventilator management, catheter care, and enteral feedings.

Monitor and document changes in client condition and report concerns promptly.

Support clients with health education, independence, and daily routines.

Collaborate with the RN Case Manager and interdisciplinary team.

Maintain accurate, timely documentation that meets Colorado Medicaid (HCPF) and CDPHE standards.

Follow all infection control, safety, and confidentiality policies.

What We're Looking For

Current Colorado LPN license (required)

CPR/BLS certification (required)

At least 1 year of nursing experience (home health or long-term care preferred)

IV certification (preferred, per Colorado regulations)

Strong clinical assessment and communication skills

Compassionate, reliable, and team-oriented approach

Valid driver's license, reliable transportation, and auto insurance
