Job Summary

We are seeking a dedicated and compassionate Qualified Professional to join our team in providing exceptional care and support to individuals across various care settings. Duties

• Assist residents and clients with Activities of Daily Living (ADLs), such as bathing, dressing, grooming, and mobility support using equipment like Hoyer lifts and resident's personal assistive devices
• Monitor patients' vital signs regularly and document observations accurately in compliance with HIPAA regulations
• Administer medications safely following prescribed care plans and document medication administration records
• Support patients with memory care needs, including dementia and Alzheimer's care, ensuring a safe and comforting environment
• Prepare nutritious meals tailored to individual dietary requirements and assist with meal service
• Conduct patient observations to identify changes in health status or behavior and report findings promptly to supervising healthcare professionals
• Maintain cleanliness of the living environment by performing routine cleaning tasks aligned with health standards
• Collaborate with interdisciplinary teams including social workers, nurses, and therapists to develop and update personalized care plans
• Assist with transportation or outings within the community to promote social engagement and independence
• Follow all safety protocols during patient handling, including heavy lifting techniques and use of assistive devices like Hoyer lifts
• Ensure compliance with all applicable regulations such as HIPAA, OSHA, and state-specific licensing requirements

Skills

• Proven experience in assisted living, home health, nursing homes, or group homes; experience working with people with developmental disabilities or autism is highly valued
• Strong knowledge of patient observation techniques and vital signs monitoring
• Skilled in medication administration and understanding of care plans
• Familiarity with dementia, Alzheimer's, memory care, hospice care, and long-term care practices
• Ability to perform CPR and first aid confidently; certification preferred but not mandatory upon hire
• Excellent caregiving skills combined with compassionate communication abilities
• Experience working within HIPAA guidelines to ensure patient confidentiality at all times
• Capable of heavy lifting and physically demanding tasks required for resident support using proper safety techniques
• Knowledge of social work principles related to eldercare or disability support is advantageous
• Competent in cleaning routines necessary for maintaining hygienic living spaces in accordance with health standards
• Effective at working independently as well as part of a multidisciplinary team focused on holistic patient well-being

Pay: $45, $55,000.00 per year
