Resident Care Coordinator- Assisted Living

Summary:
The Resident Care Coordinator assists in the overall management of resident care and wellness services within the community. This position supports the Wellness Director in maintaining staffing schedules, overseeing medication administration, and ensuring quality care standards are upheld. The Coordinator serves as a role model and resource to caregivers, certified medication aides, and other wellness team members, helping foster a culture of compassion, teamwork, and clinical excellence.
Essential Duties and Responsibilities:
Clinical Support & Oversight
- Assist the Wellness Director with daily clinical operations to ensure all care services meet company standards and state regulatory requirements.
- Administer medications and treatments accurately and in accordance with physician orders and company policy.
- Provide guidance and training to Caregivers and CMAs to ensure consistent delivery of quality care.
- Communicate effectively with residents, families, and staff regarding resident status, changes in condition, or care concerns.
- Maintain accurate and timely documentation in resident medical records and medication administration systems.
Scheduling & Staffing
- Create, manage, and adjust schedules for CNAs, CMAs, and other care staff to ensure appropriate coverage for all shifts and resident needs.
- Monitor attendance, call-outs, and shift coverage daily, ensuring minimal disruption to resident care.
- Collaborate with department heads and the Wellness Director to align staffing with census, acuity, and budgetary goals.
- Assist in onboarding, training, and evaluating new care associates.
Resident Care & Quality Assurance
- Participate in assessments, care plan updates, and interdisciplinary meetings as requested.
- Support wellness initiatives and ensure timely completion of assigned clinical documentation and monitoring.
- Monitor compliance with infection control, medication safety, and resident rights.
- Assist in maintaining a clean, organized, and safe care environment.
Team Leadership & Communication
- Serve as a liaison between the Wellness Department and other community teams to promote collaboration and efficiency.
- Provide hands-on coaching and support during busy shifts or as needed to maintain high standards of care.
- Participate in staff meetings, in-services, and training sessions to reinforce company standards and best practices.
Qualifications:
- Certified Medication Aide (CMA) or Certified Nursing Assistant (CNA) with current state certification required.
- Minimum of 2 years of experience in an assisted living, memory care, or healthcare setting.
- Previous experience with staff scheduling or shift coordination strongly preferred.
- Strong organizational and communication skills with the ability to multi-task and prioritize in a fast-paced environment.
- Compassionate demeanor and commitment to exceptional resident care.
Preferred Qualifications:
- Experience in a supervisory or lead role within a clinical or senior living environment.
- Familiarity with electronic health records (EHR) and scheduling software.
- CPR/First Aid certification.
Physical Requirements:
- Must be able to stand and walk for extended periods.
- Ability to lift, carry, and transfer up to 50 pounds.
- Must be able to assist with resident transfers and respond to emergencies as needed.
- Flexibility to work weekends, holidays, and varying shifts as required.
Job Type: Full-time
Pay: $40, $50,000.00 per year
