Job Description:

The Outreach Specialist engages the community in HCV/HIV outreach and linkage to care, with an emphasis on populations experiencing homeless and/or substance use disorder. This position collaborates with other team members in finding persons needing HCV treatment, building relationships with those being served, and addressing a broad range of their health and human service's needs.

Key Responsibilities:

  • Provide leadership for targeted outreach to individuals at high risk for Hepatitis C and HIV, including those experiencing homelessness, substance use disorder, or co-occurring behavioral health issues
  • Engage in street outreach, meeting needs and building supportive relationships with participants
  • Provide peer recovery services and support for participants
  • Provide Rapid HCV/HIV/STI testing for participants
  • Provide linkage to care for HCV/HIV/STI positive individuals
  • Establish both formal and informal alliances with organizations which have similar or allied goals to build diversity
  • Works with the Project Coordinator within designated areas to promote and connect the unhoused and community of people using drugs
  • Attend and participate in all mandatory meetings
  • Ability to build and maintain rapport with individuals who need linkage to services
  • Provide current FAN services and resources to individuals when necessary
  • Document services in a timely manner
  • Assist in reporting data through reporting gateways and submit all program outcomes to the Project Coordinator
  • Maintain confidentiality of participants in accordance with state and federal guidelines (HIPAA)
  • Represent the organization in a professional manner in all regards
  • Maintain knowledge of all FAN programs and national trends in substance abuse prevention and treatment, including harm reduction
  • Be supportive of all paths to recovery
  • Set and manage personal and professional boundaries effectively
  • Perform other duties as assigned

Qualifications, Skills & Experience:

  • Minimum of 3 continuous years in recovery
  • High school diploma or equivalent
  • Comfortable working in mobile, field-based settings and adapting to variable environments
  • Must have a valid driver's license, have access to reliable transportation, and be insurable under the organization's policy
  • Must pass a background check and sex offender registry check
  • Must pass a drug screen within 48 hours of being offered a position
  • Knowledge of Microsoft Office (Excel, Word, PowerPoint) and Google Workspace (Drive, Sheets, Gmail, Calendar)
  • Ability to lift up to 25 lbs., and sit, stand, bend, climb, carry, and/or reach frequently and for extended periods of time.

Job Type: Full-Time, up to 40 hours per week

Pay: $21.00-$24.00 per hour

This position is grant-funded and subject to the funding, budget, and timelines within grant provisions.

Schedule:

Monday through Friday during business hours, with some weekends.

Location:

Office Building / Mobile Clinic

Wayne and Macomb Counties

Experience:

Peer Recovery Coaching: 1 year (Preferred)
