Job Title: Program Director – Program of All-Inclusive Care for the Elderly (PACE)

Community PACE at Home is a family-owned business located in Newaygo, MI. Interviews for this position will be held in Grand Rapids, but this position will be in Newaygo.
Position Summary
The Program Director provides overall leadership, direction, and operational oversight for the PACE organization to ensure the delivery of high-quality, comprehensive, and coordinated care to frail elderly participants. The Director is responsible for program compliance with federal and state regulations, fiscal management, strategic planning, staff development, and ensuring the program's mission and values are upheld.
Key Responsibilities
Program Leadership and Administration
- Provide day-to-day leadership and management of all PACE operations, ensuring participant-centered care and adherence to the interdisciplinary team (IDT) model.
- Develop and implement policies and procedures in compliance with federal (CMS) and state PACE regulations.
- Collaborate with executive leadership to develop strategic plans, performance goals, and quality improvement initiatives.
- Ensure effective coordination among clinical, social, and administrative departments.
Regulatory Compliance and Quality Assurance
- Oversee program compliance with all PACE regulatory requirements, including participant rights, care planning, and reporting standards.
- Partner with the Quality Improvement (QI) team to monitor outcomes, conduct audits, and implement corrective action plans.
- Prepare for and participate in CMS and state oversight reviews, surveys, and audits.
Financial and Operational Management
- Manage the program budget, ensuring effective allocation of resources and cost efficiency.
- Review financial and utilization data to ensure sustainability and compliance with capitation rates.
- Work collaboratively with finance and contracting teams to negotiate and maintain vendor and provider relationships.
Team Leadership and Development
- Lead, mentor, and evaluate interdisciplinary team (IDT) managers and staff to promote a culture of accountability, collaboration, and continuous improvement.
- Ensure appropriate staffing levels, staff training, and professional development to maintain high standards of care.
- Foster an inclusive, mission-driven work environment aligned with organizational values.
Participant and Community Relations
- Ensure participant satisfaction and engagement by maintaining open communication and responsiveness to participant needs.
- Represent the PACE program in the community and collaborate with healthcare providers, community organizations, and regulatory agencies.
- Support marketing and enrollment efforts consistent with program capacity and eligibility criteria.
Qualifications
Education
- Master's degree in Health Administration, Nursing, Social Work, Gerontology, or a related field preferred.
- Bachelor's degree required.
Experience
- Minimum of one (1) to three (3)years of management experience in healthcare or human services, preferably with frail or elderly populations.
- Experience in PACE, managed care, long-term care, or integrated health delivery systems is strongly preferred.
- Demonstrated success in operations management, compliance, and team leadership.
Licensure/Certification
- Depending on state requirements, it may require licensure as a Registered Nurse, Social Worker, or Administrator.
- Must meet all regulatory qualifications as a PACE Program Director under CMS and state guidelines.

Pay: $120, $140,000.00 per year
