Job Summary

The Registered Physical Therapist (PT) provides professional home health physical therapy services to patients on an intermittent basis, following the physician's plan of care and working under the direction of the agency's clinical management.

The PT will:

  • Evaluate and treat patients according to individualized care plans.
  • Document condition, progress, and outcomes.
  • Communicate effectively with patients, families, and interdisciplinary teams.
  • Promote patient independence and well-being through education and support.

Qualifications

  • Active Ohio RPT License
  • CPR Certification (BLS or ALS accepted)
  • Meets all state and federal employment requirements
  • Strong clinical assessment and documentation skills
  • Excellent communication and customer service abilities
  • Valid driver's license, auto insurance, and reliable transportation
  • At least one year of home care experience preferred

Schedule

  • Flexible: Create your own weekday schedule
  • Standard Day Shifts (Monday–Friday)
  • Weekend availability optional
